We do the following:
deny !senders = cdb*@;DB/nocallout.cdb
!dnslists = dsn.rfc-ignorant.org/$sender_address_domain
!verify = sender/callout=3m
The locally-maintained exemption list contains only a few domains;
rfc-ignorant deals with the majority.
