Thanks Odhiambo and John, your hint was helpful. My old configuration
file came from a previous exim3 installation which had not been
completely removed.

I suggest to add a "note for Debian users" to
http://www.exim.org/howto/mailman21.html
For example something like:

Note: Debian has a complex system to make debconf automatically
generate the exim4 config file. Before applying the changes described
in this document you should read `man update-exim4.conf` and
related Debian man pages.
