[exim] Host lookup process

Can someone point me to a description of how Exim performs host lookups for
resolving domains and/or mx servers, complete with information as to what
would cause the process not to complete?

I have a customer who can't email their attorney because exim can't complete
the host lookup for the attorney's domain.

I've done it manually, and it seems to work fine.
