Hmm. I've tested both your first and your last suggestion both by
editing /etc/exim4/update-exim4.conf.conf and by entering the
value in the debconf frontend and both just work for me.
Have you implemented the change with /etc/init.exim4 restart? What
does
/usr/sbin/exim4 -bP local_interfaces
say, and where oes the configuration come from
/usr/sbin/exim4 -bV | grep ^Configuration
