We had the same trouble around authentication and no amount of debug
turn-on helped.

Our trouble turned out to be that we were authenticating the local
part only instead of local_part@domain. This was because we didn't
specify a default_domain in the authmysqlrc file.

We had to compile more debug outputting right into courier-imap to
figure this out.

And it was something so simple.

BTW, the default for courier-imap logging is /var/log/syslog.
