Actually, you'd have to add your username to trusted_users or mutt still
inserts a Sender: header that MUAs like Outlook and Eudora love to
grab hold of ...

Or you have to find a mail client that talks smtp to exim (thunderbird
etc, or maybe pine with smtp server set to localhost)
