Answering my own question. turns out, my /var/mail directory was
writeable by the courier user, not exim. Adding:
user = <courier_userID>
group = <courier_groupID>
to my vacation_autoreply caused exim to create and use the vacation.db
files automagically.
Yay Me! :)
