I have just been bitten by having the "accept authenticated = *" really
far down in the check_rcpt default ACL (4.32 split config). I have a
bunch of [HE]LO checks[1] about midway and it was rejecting users.
Typically end-users don't set their machines with an FQDN and without
one I'd reject mail. Authenticated SMTP should allow it though.
Would be nice to have that further up.
