Exim versions prior to 4.x are seriously out of date and should be

This is either:-
1. Exim is doing a reverse DNS lookup on the connecting IP address
and failing. Check the host_lookup option. DNS lookups may
also be force by having hostnames in host lists that restrict
connection/relaying etc. Check this by doing "dig -x i.p.a.d"
on the exim box - you may wish to fix any DNS problems seen.
2. Exim is doing an ident lookup which is being blocked or
otherwise timing out. Check the setting of
rfc1413_query_timemout and maybe set it to zero.

That tends to point away from the DNS answer.
