Is it virtual users in MySQL or real local users that you're having a
problem with? Your userforward director has a check_local_user option, which
means it will only fire if the address being directed is a local unix user -
won't work for your virtual users in MySQL.

If you want to make .forward work for virtual users, you'll need another
director which determines the 'home' directory (ie where to find the
.forward file) for a virtual user (from the db, or by deduction from the
address, doesn't matter), and triggers if the file is found.

Oh, and please think about upgrading to Exim 4.
