You basically lose the ability to use RBLs if you get the messages
forwarded onto you by some other service, or handled by some other
delivery mechanism.

You theoretically could parse the sending ip out of the received headers
and pass that on to exim as a command line attribute, but its probably
rather more trouble than its worth (also be very careful about bouncing
stuff as fetchmail like system can have odd side effects).

Its probably just easier to push everything through SpamAssassin which
should do RBL checks on received headers and use the scoring as a
routing decision of some form.
