I'm not an exim expert, but I had something like this occurring on my
system at one point. Turned out to be a result of my .forward file. The
message was getting rejected in the RCPT ACL the first time through,
then being rejected again by the RCPT ACL after the forward, causing
double rejection entries in my exim main.log. Still not clear on what I
had misconfigured to cause it to be forwarded after being rejected, but
possibly something to look at in your set up...
