If the system filter didn't finish - presumably because the system
crashed or the power failed or the process got chopped or something -
then Exim will not have updated the spool file for the message, and so
it will still be in "first_delivery" state next time.

In other words, the "first_delivery" flag gets unset only if an Exim
delivery process gets through the delivery attempt and ends up with
some addresses that it has to try again later, and then successfully
updates the spool file for the message.
