JFYI,
The Debian packages come with exiscan included. You might want to check
what flavour of the exim4-daemon package has this exiscan patched in,
but afaik all except the light version have exiscan.
Exiscan also gives you virusscanning, malware(?)scanning, and other
neat tricks.
I prefer sa-exim though, a project that implements spamassassin in
exim's local_scan() feature[1].
