Many also use non-zero gif/jpg's to do the same.

Answer to that really belongs to the MUA the user uses.

With MUA's that support it, before they open anything not included in the
email they either prompt or don't do it so that if the gif is in the mail
file your were sent you would see it, but if it had to fetch the gif, you
would not.
