I am thinking, and I could be wrong, that you might want to use:
server_condition = ${if saslauthd{{$2}{$3}{smtp}}{1}{0}}
^^
instead of:
server_condition = ${if saslauthd{{$1}{$3}{smtp}}{1}{0}}
(For the same reason that you use server_set_id = $2, instead of $1)
