Ok, that got me a little farther - but now I get:

454 TLS currently unavailable

Here's my new exim4.conf section:

hostlist relay_from_hosts = /etc/exim/relay_from
hostlist host_auth_accept_relay = *
tls_certificate = /etc/exim/mail.3e.org.crt
tls_privatekey = /etc/exim/mail.3e.org.key
tls_dhparam = /etc/exim/mail.3e.org.dh
tls_advertise_hosts = *
hostlist auth_over_tls_hosts = *
