The clamd process doesn't have permission to inspect files in Exim's
spool.

Check what group owns /var/spool/exim, then add the clamav user to this
group in /etc/group and make sure you have this in your clamav.conf:

User clamav
AllowSupplementaryGroups

Obviously, you may user a different username.
