O'Reilly declined to publish the second edition. Luckily, a very small
company here in the UK agreed to take it on. I suspect it wouldn't have
got published at all otherwise. Unfortunately, it is not easy for a
small UK company, unknown to the main bookstores, to persuade the big book
chains in the USA to take on a rather specialist book. The timing was
bad too, with all publishers and bookstores suffering a huge downturn (I
read somewhere that some publishers had 50% of their books returned by
bookstores).

I know that UIT have been trying hard to improve availability in North
America; I don't know exactly what the current status is.

Anybody (individual or bookstore) *should* be able to order it. The web
page (see below) does have an email address for bookstores who want to
order for resale.
