Exim cannot replace headers inside acl's, you must do that on the
system filter, i.e:
if $h_x-spam-score: contains "++++++++"
then
headers add "New-Subject: [SPAM] $h_subject:"
headers remove subject
headers add "Subject: $h_new-subject:"
headers remove new-subject
headers add "X-Spam-Flag: YES"
endif
