I guess I can read the code faster than you can.... :-)

Looks like per address. It works like this:

. The domain is remembered.

. The address is routed.

. If the router has same_domain_copy_routing set
  AND there are no headers to be added or removed for this address
  AND the domain was not modified (widened) by the router (by comparing
      with the old domain
  THEN search for any other addresses with the same domain and copy the
       routing
