[Exim] Why am I breaking HTML mail?

Configuration:

    Exim 4.14
    SA-Exim 2.2(for Spam/virus) transport rejects
    Exiscan .26
    Sophie 1.43

I think that I have disabled all of the de-mime /defang options in the
various components, yet some users are still seeing non-spam broken HTML
mail. Anyone have some hints?
