Tests seem to indicate that this problem is solved in exim v4.14
(no problems in over an hour with the same load)

Oh! Sorry! My apologies. I had forgotten that it was only at 4.12 that I
modified -bV to output more details about what the binary contained. You
will have to use
exim -d -bV
to get it to tell you which db library it is using.
