Means that the subprocess running a filter failed in some way, so didn't
produce the expected output.

How do the contents of a "working" and a "non-working" foward file
differ?

Hmm. That isn't even a filter file, in which case, it shouldn't be
running a subprocess. Are you sure that's the right file?
