(1) Via the local_scan() interface.
(2) You could run a perl function as part of a "headers add" or "headers
remove" command in a system filter, I suppose. This isn't really a
general-purpose "alter", however.
(3) Similarly, you could do the same for a headers_add or headers_remove
option on a router or transport.
