It would be useful to see this header.

Have you tried on an Exim 4.12 without exiscan or spamassassin?

That is a crash very, very early in the delivery process. It does indeed
suggest that the header file is damaged in some way. Please send me a
copy of the -H file off list, and I will see if I can spot what the
problem is.
