Make sure you have perl installed and your spamassassin perl modules are
installed.

for good measure ( my mirror I was connected to did not have the
spamassassin modules )

then download from
http://marc.merlins.org/linux/exim/sa.html the sa-exim
source. Copy the sa-exim.c file over the source of the src/local_scan.c
(of exim 4.1x source).
Compile, install. Configure your spamassassin settings and if your
exim.conf file is ok then any email will be scanned as they arrive, and
rejected(if spam hits >= spam score) real time.
You should not need to really set any thing in exim.conf for spam assassin
when you do it the sa-exim way.
This is how I did it.
Also Exim if patched can run loadable modules. if your exim is patched for
this you would not need to recompile. I dont know if there are rpms for
this? again refer to Marc Merlins website about this.
