Many thanks to everyone that assisted, especially Philip.
The problem was the lsearch in relay_to_domains.
Exim was not reading the rtdomains file properly.
It works now.
LOG: H=(bob.co.za) [100.50.3.250] F=<iuqhef@???> rejected RCPT
johannes@???: relay not permitted

The address it is coming from is indeed 209.212.109.219. That is why I
suggested you try a test with
exim -bh 209.212.109.219
which simulates a connection *from* 209.212.109.219. Details in TFM.
