Have you run this test though in debug mode?

exim -d -bv pest@??? (checks recipient address)
exim -d -bvs some.person@??? (checks sender address)

Will give you lots of detail...

You can also check through section 5.3 of Exim 4 spec document for more command
line options for debugging.

This may give you enought to pin down problem.
