It would do that if it crashed while in local_scan(). I presume you are
using that interface? If the call to local_scan() times out, Exim should
remove the -D file. I've made a note to run a test to make sure that
that code does actually work. In that event, it should also write a line
to the main and reject logs recording the timeout.
