smtp-server = smtp.example.com/user=foo (I think pine only does AUTH
LOGIN...)

Even speaks TLS now.

Only thing - the password can't be stored - it is cached for a while after
which you have to re-enter it again.

If you use pine anyway best way is to have it talk to a local exim, which
smarthosts through a remote mailserver using AUTH.
