The same way you were adding users for sendmail -

/etc/passwd entries?
LDAP?
MySQL / PostGRESQL?

Take your pick.

As for "how does a user pop into their account" he uses whatever pop3
capable mail client he likes.

Assuming, of course, that you have a pop server installed and configured.
