But you haven't omitted both strings. You have got an empty string ({}).
That is what exim has printed. Try

testprint "${run{/usr/local/bin/mytest}}"

and then try

testprint "${run{/usr/local/bin/mytest}{XXXXX}}"

to see the difference.
