It happened using Exim 3.35 on Linux 2.4.19.
This mail was bounced to two different addresses using mutt. Here's the

What does "remote delivery process count got out of step" mean?
This bounced mail was not logged to be delivered but it was indeed
delivered to the destinations. Here are two Exim logfiles from the
recipient's mail servers stating having received the mail:
