My suggestion would be to simulate the nessus test using "exim -bh" and read
the messages to try and figure out why relaying was allowed.

sth like

exim -d -bh 1.2.3.4
EHLO testhost.example.com
MAIL FROM:<>
RCPT TO:<not-local@???>

You didn't run nessus from one of the IPs listed in host_accept_relay, I
hope...
