Is that use of ".*" on the LHS triggering a switch to use of regular
expressions?

(.*)\.\.(.*)@REAL_DOMAIN $1/$2@REAL_DOMAIN Tbct
does that help?
I can't immediately see anything in section 30 of the Exim 4 Spec which
describes the use of regular expressions, but the last example, for
bang-paths, does make use of the feature.
