BTW: I haven't tried spamassassin yet, but in my opinion at least the
amavis-condition must be changed to be ignored when received_protocol
is spam-scanned _or_ scanned-ok to avoid loops.

Something like:

${if or {{eq{$received_protocol}{spam-scanned}}
{eq{$received_protocol}{scanned-ok}}{0}{1}}}
