Found it:

After more testing, I found that even if you put the IP address in the
"host_lookup" line to exclude, the "host_reject_recipients" will cause it to
fail again! You have to add an entry to this table also. (!xx.xx.xx.xx/0).

Could this be considered a bug possibly? Shouldn't the exclusion in
host_lookup cause all further tests on the IP address to pass?

That makes me feel better, I tried that entry in the "host_lookup" line,
(that is right isn't it?), and still I get a "recipients from
[xxx.xxx.xxx.xxx] refused (failed to find host name from IP address)" error.
(I removed the IP address intentionally). I suspect that the reverse lookup
is failing in another check.

I have been tempted to add the IP address to my own host file so that the
reverse lookup will pass, but don't feel that this is the correct way to fix
this problem. I would prefer for all of my exim configuration settings to
remain in the configuration file if possible.

If you have any other suggestions, please pass them on. In the mean time, I
am going to do some more testing to see if I can find the exact cause of the
failure.

The hard, we can do quickly, but the simple stuff will always stump us for
hours.
