I wish I knew. It keeps getting reported from time to time, but nobody
has ever come up with a foolproof way of reproducing it. Consequently,
I've never been able to track down what is going on.
Check the permissions on /var/spool/exim/db/wait-remote_smtp.lockfile
and all its superior directories.
