Broadly I'd like to be able to implement a sendmail milter like
interface here - in fact I have considered the idea of really making a
milter interface (probably limited since I understand the sendmail one
lets you hack sendmail internal variables as well).
So that would mean hooks to allow:-
- add/remove/modify header lines
- replace body
