Is the one remote client connected to your site perhaps via an
internal/compan wan connection, whereas the other two are via the
Internet?

If thats the guess, the pix is 100% the problem. It blocks ESMTP by
default for no good reason. A server I admin is behind a Pix.. luckily,
I am also in charge of the Pix. The first thing I did when I took over
running it was to "no fixup protocol smtp"
