What exactly where you doing? Is this error reproducible? Which version
of Exim and which operating system?
What this message means is that Exim was delivering a message to a
number of different remote hosts in parallel sub-processes. It thought
it had n sub-processes running, but when it came to wait for them to
finish, they weren't all there. It's not a problem I have ever seen
before.
