Grepping 'man cat' (at least on FreeBSD) gives:

  DIAGNOSTICS
       The cat utility exits 0 on success or >0 if an error occurred.


So, you're cat is failing.

Run it from the command line, to see what it's complaining about.

Or turn on return_output.
