two more data points that i should have included in the original
email:

1) exim 3.33 on redhat 7.1

2) the connection for the spam that leaked thorugh was a TLS
connection (i don't know the internal flow, and am only speculating
that there might be a path by which an inbound email coming in over
TLS might circumvent the rbl_domains check.)

i have added the particular IP range that leaked to hosts_avoid_tls to
see if that makes a difference; the particular spammer is the
milespree.com bozo from trancentives.net, and they spam often enough
that i'll find out if this is the issue pretty soon, i think.
