Odd. Have you looked to see if there is anything in Exim's paniclog
file?

Not trivial. I suppose one way would be to set up an alias for a pipe to
a script, and make the script do "sleep 30" so that it keeps a message
locked for some time. Then send a message to that alias (using -d9 if
you like to see what is happening). When that delivery has got to the
pipe and is sleeping, try running "exim -d9 -M <message-id>" to attempt a
second delivery on the same message. You should get the error

"Spool file is locked (another process is handling this message)"
