It works for me, the only thing different in my tpop3d.conf is that I
don't have the line "append-domain: yes".

Depends on how much you trust your mysql server. If it's down, no mail
can be deliverred.
If you want to do it, you could try vmail-sql, a setup for exim +
tpop3d + mysql, from the authors of tpop3d:
