As well as the useful comments others have posted, another outside
possibility could be that something is tying up all the incoming SMTP
connections (as set by smtp_accept_max). Have you tried using exiwhat to
see what Exim processes are running? And netstat to see what connections
there are, of course. Just a wild thought.
