If you want to match exactly that, you need
if $message_body BEGINS "Dear friend"
If you want to ignore case use
if $message_body begins "dear friend"
If you want to ignore blank lines and spaces before it then
if $message_body MATCHES "^ *Dear friend"
or
if $message_body matches "^* dear friend"
