No, it doesn't. If I added "in the main configuration" to every
reference to every main option in Exim, the manual would not only be a
whole lot bigger, but it would read very clumsily. However, in this case
there is already a scribbled note in my copy of the manual to clarify
this information, and I'll try to make it clearer in the next edition.

Note also that there is a file called doc/OptionLists.txt in the Exim
distribution which contains a complete list of every option, and this
tells you that message_filter is a "main" option.

Think about it. If you had two filters, how would Exim know which one to
run? If you just want them both run, why bother to have two? Just stick
the filter instructions consecutively in one.

Yes. But you can stick "noerror" before the command to cause errors to
be ignored.
