That is quite an expensive option, because it has to go through the list
of current connections and find out which ones are for the incoming
host (strictly, IP address). It does do this before forking, to prevent
a rougue host causing too much forking.
What value have you set for smtp_accept_max? If you can reduce that, the
testing should speed up.
I've made a note to think about whether the way that the check is done
could be speeded up in Exim 4.
